Everyone! Children & families receive better communication and support, have stronger relationships with providers and save time. Children and families who have special healthcare needs, behavioral health needs, or require multiple medical professionals will benefit from additional support and/or care coordination in a PCMH. We have dedicated Care Coordinators at our clinic to help families with community resources and assistance, referrals and collaboration with specialists, or general questions about a child’s plan of care.
Specialist Referrals – We are available to help facilitate coordination of care for different specialists when patients are referred by their pediatrician. We organize and share appropriate information to those involved with patient care to provide continuity and quality care.
Resources – We work to keep accurate community resource information to provide our patients and their families with current resources and assistance such as: transportation options, food assistance, mental and behavioral health care, Developmental Disability services, WIC information, insurance assistance and medication assistance.
Diabetic Clinic – We partner with Portneuf Medical Center’s Diabetic Education Department to provide quality care for patients with Type 1 & Type 2 Diabetes. We offer quarterly Diabetic Clinic appointments where your child’s pediatrician and nurse sees you in conjunction with the Diabetic Educator and Care Coordinator to download and review your meter, CGM, (Continuous Glucose Monitor) and Insulin Pump. This will help determine any changes that need to be made to your child’s insulin regimen and this is also a great time to ask your Care Team questions and share any concerns you have. Together we will work together to provide your child with the best quality of care!
Telehealth – We partner with the pediatric neurology team at Primary Children’s Hospital to offer monthly telehealth visits. Patients can come to our clinic and visit with a pediatric neurologist via secure video connection to update medications and get answers to questions. Hopefully saving families time and money by providing local access to high quality neurological care without having to travel and take time off work and school.
Transitions – Our Care Coordination Team is available to assist during transitions of moving, or changing to an adult primary care provider. We will help to schedule a time to talk with your pediatrician about the transition and provide options or recommendations for who will take over your child’s care at this next stage and how the transition will happen, with your child’s best interests and your preferences at the forefront.
